1. Yes it needs to be full when the inspector arrives. You will have to fill it.
2. You can just cap the pipe before it ties into the main drain line. Here would be a good place to install a boiler drain so that you can let the water out using a garden hose to keep from making a mess.
3. Yes, each fixture rough in will need to be plugged.
4. You could install a cleanout tee in the new line just before it enters the main drain, using an inflatable screw in test plug in the tee would allow you to go ahead and tie into the main line, water test your new dwv and continue to use the first floor bath.
This would probably be your best option, then after the test all you need to do is deflate the test plug, and the water will go out the sewer line and you won't have any mess to deal with and the final connections will have already been made. One of these inflatable test plugs would be an expensive item for a one time use. It would be handy if you could borrow one from a plumber friend if you happen to know one. Or if you buy one you could resale it on an online auction site. I have bought several there.
